Will my insurance cover my visits?
Dr. Miletich is considered an out-of-network physician and also does not participate in Medicare. Surya Medicine does not bill any insurance but can provide a superbill with appropriate CPT and ICD-10 codes which can be submitted to insurance companies for reimbursement.
Will my insurance cover my labs?
This depends upon your insurance coverage and the labs ordered. Surya Medicine will submit a lab order along with appropriate diagnosis codes needed for coverage. It is best to check with your insurance company prior to having labs drawn for clarity about coverage.
How much does a visit cost?
Visits are billed by time as follows:
-15 minute visit $95
- 30 minute visit $190
- 45 minute visit $285
- 60 minute visit $380
Other services such as pellet insertion, IV therapy, O-shot, IUD insertion, endometrial biopsy, etc are billed separately.
Are telemedicine visits available?
Yes, Surya Medicine offers both video and phone visits. The price is the same as for in-person visits.
Does Dr Miletich perform female annual wellness exams/gyn exams?
Yes, Dr Miletich does female wellness exams including pap smears. Visits are based on time.
Does Dr Miletich place/remove IUDs?
Yes, we use your insurance coverage to order the IUD. The device is then mailed to the office and we can insert it during or just after menses. It is a good idea to contact your insurance company to inquire about coverage of the IUD as well as coverage of IUD insertion by an out-of-network provider.

Can Dr Miletich be my primary care physician?
No, it is better if you have a primary care physician to help you with urgent and after-hours care. Dr Miletich is only available for scheduled appointments.
How do I schedule a visit?
Hit the "request an appointment" button on the website and provide us with your first and last name, email, and phone number. A member of our team will reach out to you to schedule an appointment. Once an appointment has been scheduled, an email will then be sent to you with a link to set up a new patient account. This will allow you access to our portal, where you can complete the new patient medical questionnaire and other necessary forms before your first visit.

What can I expect during my first visit?
The first visit is the time when our health care providers get to know you and your health goals. We would like to have all pertinent medical history/records prior to your first visit. *Please allow 45 minutes to 1 hour for your first visit.*
Please send in relevant medical records and lab results from the last 12 months. Please DO NOT send pages of notes from routine medical visits or lab results that are over 1 year old.
What is your cancellation policy?
We understand that there are times when you miss an appointment due to unforeseen events. However, when you do not call to cancel an appointment, you maybe preventing another patient from getting needed care. For this reason, we have a 48 hour cancellation policy. If appointments are cancelled less than 48 hours before the scheduled appointment time, there will be a $50 fee due immediately upon cancellation.

It is best to schedule an appointment for a blood draw.
Where should I order my supplements?
Surya Medicine has an online store for patients to use. You can access it through the portal. You may also go to MDprescriptives.com for some supplements.
It is important to only use high quality supplements that adhere to certified good manufacturing practices (cGMP). Be careful of third party vendors and big box retailers. Getting supplements from the right source is important to ensure the highest potency nutrients.
What do I need to do before my first appointment?
Please fill out the medical questionnaire/consent forms. These can be found on the portal. If you have relevant medical records and lab tests (from the last 12 months) to share, please send them in prior to your visit. You may send a message to us through the portal and attach your medical records to the message.
Having all forms completed and records available prior to your visit allows our health care providers to have more time with you.
